You can add an entry to a form:
- Click Forms in the toolbar.
- Select the applicable form folder in the tool menu.

Select the applicable form in the content pane.
- Click Add Entry.
The applicable form field appears.

Adjust the setting(s) of the form field.
- Click Submit
Pop up windows inform you that the entry is added and the form entry is submitted.
- Click Add another entry, or click the X icon to return to the selected form folder.
